As long as your GPA doesn't drop you're definitely in at NYU. If you bring it up, good chance at CC too probably. Uva and Penn might YP you, in at Michigan, out at Berk. In at Cornell/Duke/NU/GT too.
Pretty sure you don't get a URM boost, but you have decent softs.

Also, I would edit out your post if I were you. It's super identifying.
You should also be careful about discussing mental health issues in an addendum. That could open up a whole can of worms and could unfortunately cast you in a negative light. Law school is stressful, and you don't want to give adcoms any reason to think you're not up for the challenge.
