3.5 and 166 w/ huge gpa jump

I was diagnosed with a learning disability after my So. year of college. My GPA for my first two years was 3.3. My Jr. yr GPA was 3.97. Overall, it's a 3.5. I'm expecting another strong semester. Possibly 4.0.

What are my chances for...
University of Maryland
George Mason
George Washington

Should I wait to apply until my GPA goes up in December or apply now?

Will my upward trend/an addendum explaining my GPA increase really help my chances?

apply higher. apply to schools in the wustl/emory/fordham/bc/bu/uiuc/nd range.
