You'll get more responses if you post into the general forum. My guess is most of the '16ers won't be checking this forum at point, and many of the '17ers are aspiring applicants.
That being said, I can offer two data points gleaned from the '17 thread:
- No (myself)
- Yes (clouded.memory)
As to your other question, no Top 10 (or T-14 for that matter) has this "requirement".

Accepted without targeted letters two cycles ago.
Unless your recommender has specific and personal knowledge of SLS and can credibly argue why you and SLS are a particularly good match, a targeted letter isn't going to do anything. If the letter will just include boilerplate restatements of crap from the SLS website or, worse, just substitute "Stanford Law School" for "law school" in the non-targeted LOR, don't bother.

mine weren't targeted; no t10 has the requirement, and only sls makes the suggestion.
