I believe it is frowned upon to tell more than one school you will definitely go to their school if you are admitted.

Happens fairly often, at least with personal statements. Not sure if the same is true for the optional essays. I would just tell them that you added a previous version of the document, and to please add the updated one.

I did the same thing last year and was admitted into all the schools LSP said I would be admitted. Don't freak out, happens pretty often. Look at the bright side, if they do accept you, that will be a pretty funny anecdote.

Just let it go and see what happens. I doubt schools read LOCIs very closely, so just hope no one notices. Nothing you can do about it in retrospect.

I recently made a similar mistake and found out this week I was admitted anyway. Giant caveat: this school's a TTT.

the adcomm is typically comprised of humans. I would think that the biggest reaction your mistake will elict is a chuckle and a muttering of "poor, dumb bastard." But I really don't think they will give you any less consideration based on just that. If your numbers are good, I wouldn't worry.
