From what I was told by Dean Hoye, they only sent the form to the very initial waves of PT admits, so there's no need to worry right now.

Just wanted to say good luck all, and if you end up on priority reserve or waitlist don't despair - it's not a graveyard. If you really want Duke, and you make that abundantly clear to them, they take will take it into account!

Not sure what's going on with my application. Back on 10/07, the status checker said my app was complete and was now under review. But today, it says just file under review with a date change for 11/13, and the text at the top says "Your application will go through several reviews. The process is very intensive and may take extra time to complete. We are unable to provide an exact date of when you will be notified. A decision could be made at any time between now and the end of April."
I assume before today it was just in the 'complete' stage, and only now is it actually in the 'under review' stage, but at this point, I'm giving up on getting useful information from any status checker.
