Since there's still no Michigan thread I'll ask here: To solicit a fee waiver should I send them the form present in their application on lsac, which seems for need based waiver, or is it possible just to send an email like everywhere else?

I applied to a number of schools last cycle and most, if not all of the schools I was accepted to wanted an answer/seat deposit by April 15th. With that being said, I got the impression that if would have sent in a seat deposit in May or even June any one of the schools would have gladly accepted it. So I think my LOR writers forgot about me... I asked them in early June and initially asked for the letters by the first few weeks of August (when I was still optimistic that I would apply with my June score). August is almost over and I still have no LORs... it's time to send a follow up email, right?
