I didn't because I (hope I) was able to convey that in my interview. The hold letter said we could submit a LOCI, but I am not sure if that is necessary. I didn't either because I thought ED plus going to Chicago to do the on campus interview (and saying why in it) would be enough. That seems like a form that would be incredibly easy to miss if you are let down from the letter and upset.
