This might be a dumb question, but does "under review" literally mean they are sitting around reviewing my application at the moment and going to make a decision by the close of the day? Or does it just mean they've got everything and its in the pile along with everyone else?

You can be under review somewhere for hours, days, weeks, months, there's really no telling.
Under review just means that they've received all components of your application and it is in the possession of the admissions people who are making the decisions.

Yesterday I got my Duke decision and when I got the "Decision made;watch for email" notification on the status checker I lost my mind refreshing my email for the next hour until I got my decision.

I even paid $30 for a taxi back home instead of taking the train because I didn't want to lose service

But you could have spent that $30 on CAS!
