So like mail it? I don't know how else to get it to them. I did it twice - once I filled out the PDF electronically, and once I printed it, filled it out, and scanned it... Any other tips?
Mail or fax it would be my guess, since there is the fax number and address on the form. It doesn't have super strong instructions, I'm just mailing mine.
