1.) A LOCI can be a Why Duke. Just start it with "I am writing to express my continued interest in attending Duke Law. 3.) About a page, I'd think. I would make it the body of your email, and end with "the contents of this email is attached as well" or something like that.
4.) If you have a high chance of attending the school, then it is among your top choices, so say that. Schools know that it comes down to other things. In fact, if it were your ABSOLUTE top choice, you would have EDed. So just say it is among your top choices. You shouldn't be going on and on about how much it is your top choice, instead you should be writing WHY it has that place.
