Correct. Plan on interviewing. There have been exceptions, but they are definitely exceptions and not the norm.

Is an LOCI only appropriate post-decision or can one be sent if an applicant has not hear word for three months and counting?

Only appropriate after being waitlisted. Your continued interest is implied if you have not withdrawn your application.
