I don't think it's a great idea posting exact questions here -- if everyone starts asking the exact same Qs, it won't be too difficult to figure out where the info came from. Sounds to me like interviews go primarily to applicants who are extremely good candidates for admission. They definitely don't interview everyone.

Almost definitely. I have 2 friends IRL who are students at WUSTL and neither of them interviewed - unless they've chosen to lie to me for some bizarre reason. I'm guessing your friends are 2/3L's? As of last year, I'm pretty sure an interview in mandatory for acceptance.

I had the same experience as OP.
