Heading on Personal Statement.

What should the head on the personal statement be?

Any suggestions.

It should have your social security number and your name on every page.

As to the title, I just called mine "Personal Statement"--Not too creative, I know...

name/ssn in the header section is all I'm doing with reference to whatever number or section in the application it applies to

Do you need a header on the personal statement? or can I just name the file "first last PS Lsac #"

I went with a 3 line header with:

first last
L12345678
Personal Statement

Replace "Personal Statement" with "Blah Blah Addendum" as necessary. Of course, the resume did not have such a header and followed a more traditional resume format.

If I make even one more space I go over two pages.. I've been trying to cut it down for the past hour and can't!
