Answers credited discrepancy?!? Help

Got my score for my Oct test. The answers credited was 5 less than I actually answered correctly. Can someone please explain why?

Did you forget to bubble in your middle initial on the scoresheet? They deduct 5 points for that.

This happened to my cousin. He has two middle names so he didn't know what to put. Ended up complaining to LSAC and they gave him a free retake but deducted .02 from his GPA for his attitude.
