PT Data Tracking

Anyone have a good excel doc for data tracking? I've been trying to dig one up online and only found the Cambridge one, but it looks to be missing some of the PTs I have, so it won't necessarily measure the trend. I've seen some quick and simple ones, but hoping to find something a little more comprehensive.
