No addendum, you're fine. Not from personal experience, but I asked a bunch of people this question in the past and the answer was almost always no for most schools, unless there was some actual unique circumstance (death in family, whatever)

Read the application materials for the schools you apply to carefully. Some schools do state that they 'invite' applicants with multiple LSAT scores that are significantly different (8+ points) to submit a brief addendum to explain the substantial difference in earned test scores.
For most schools, if they do not specifically ask for or 'invite' one for this type of circumstance, it will probably not matter if you do not, a 174 speaks for itself. However, as an applications golden rule, if a school asks for something or invites it under certain circumstances, they want it and it's better to be safe and give them what they want rather than blow off something they have asked from you.
Your addendum would be easy, short and simple. A few lines of text at most. "I was ill with a fever when I took the October 2012 LSAT. My December 2012 LSAT score is a more accurate reflection of my true abilities."
