Cancelling Forum

Decided to cancel my score while writing the test - messed up pretty bad on silly logic game and couldn't focus. The cancel form on the back stated that "sign with your full name" - I didn't do that, I only put my first name.. when I talked to proctor talk about it she said it wasnt a big deal and that it doesn't matter. But then I checked on LSAT website and it said that all the instructions have to be followed completely.. what should I do? Am I just panicking?
