If I start studying now, is a 165+ possible?
I took a diagnostic test proctored by Kaplan and made a 157 (Test 33, or something close to that. I feel like I could have done better. I base this on the proposition that people generally do better on tests when not hungover.)
I have studied a small amount (basic/advanced linear games in LG Bible, two chapters in LR Bible).
If I start studying this weekend, five hours a day for approx. one month, is a 165 attainable? I feel like I'm a fast learner, for what it's worth.

Probably. With enough effort, I think it's possible. My diagnostic was 155. After a one month Kaplan course, I got a 165 on the real test. It's definitely possible, and I feel like I could have done much better if I took more practice tests and studied for more than a month.

I took the October 2010 test after starting dedicated study during the last 2 weeks of August. I think you may have a little bit less time than I had, but I went +12 on my diagnostic on the real thing.

Definitely possible. Don't be afraid to postpone, though, if you're not where you want to be by test day.
