Uhh, it seems weird that you need to personally remind this person yourself. Shouldn't that be LSAC's job? I mean they say they won't process it until he sends stuff in. What if he never sends it in? Do they just not do anything? I would assume they have some process in place for reminding proctors to send shit.
If you want to take into your own hands, call him. If he doesn't pick up leave a message and email. In person seems ridic.

OP, first off, just breathe.
Second, and I don't think you will like this, your scores will probably be delayed.
Harassing the proctor would be a mistake. I do not think I would even call. This is LSAC's problem and interjecting yourself into the middle of it will only further complicate matters.

I think a VERY friendly email reminder would be wise. Make sure the email contains no hint of blame. Be very apologetic. 'I don't mean to rush you and sorry if I come off that way'. Your objective is to get him/her to move his/her ass. The best way to do that is to kiss ass.

What was wrong with the booklet? It should be their job and you really shouldn't have to remind anyone to do it. However, a friendly email could ease your worry.

Damn! I probably would have ignored it unless it interfered with the passage or something. I haven't gotten a hold letter yet. I made a complaint about a protor. Do they send that to you by email or what?

Call, friendly question inquiring if LSAC had contacted them about the issue. This is your future. Who gives a rats ass if this causes a small distraction for some random proctor.

Well that sucks man! I would call LSAC on the 1st if you don't have your score and ask them how long it will take.

The proctor has no obligation to respond to you. Nagging him with a phone call may deter him from going out of his way.
