I faxed my LSAT score cancellation form to LSAC via fax today. But now I see that
it says on the LSAC website that I have to send them my statement that I wish to cancel my
score, and my SSN/SIN? (which the form did not have a space for)
Does that mean I was supposed to write that statement, together with my SSN on a separate sheet
of paper and send it to them?
or just faxing the form would do the job?

Just so you know, most forums prefer that you not bump a thread within less than 24 hours of its most recent post.
If you mean this form (http://www.lsac.org/JD/pdfs/ScoreCancellationForm.pdf), filling it out and sending it should be sufficient.
