Help: writing a deferral request letter Forum

Does anyone have any advice or tips on what such a letter should contain, how it should be worded, successful versus unsuccessful reasons for requesting a deferral, etc.? Would anyone be willing to share a sample letter? I am looking for some general--but solid--direction here.

Alternately, would anyone be able to provide links to relevant threads in which this issue is discussed? I have searched but have found nothing truly remarkable.
