I couldn't find it anywhere (just blind, apparently) and ended up calling around until a dentist office finally answered their damn phone. It was an office with multiple dentists who accept the plan, and the receptionist gave me their Cigna number. I miss my old HMO in Oregon. It made everything so easy.

Had burns for evidence. Liked him a lot. Very sharp dude and nice to boot. That said, also had him for ethics and it was REALLY boring.
