How do you ask for recommendations?

What's the etiquette for asking for letters of rec? Do I try to set up a virtual meeting or is an email sufficient?

I went with an email to start, saying something along the lines of "can we talk about career stuff," and then asked during the ensuing meeting. Seemed to work well for me.
