One of my closest friends had the vice versa experience - started at a tier 4 and transferred to a top-20. He was in the top 1-3% at the first school, and slightly below median at the second despite working just as hard. At least in his experience it wasn't only that there was a qualitative difference between the students, but also that the curriculum was different. You also have to bare in mind that OP was medicated this year, and I'd imagine doing law school with an untreated mental illness would be like playing football with a fractured hamstring. I can speak from personal experience in saying that I've had insomnia for about a decade, and while I was able to do pretty well without sleeping, my grades shot way up the one semester I took medication for it. I ultimately got off it, because I would go jogging in my sleep, which is funny in hindsight but was less so when I woke up a mile from home with no wallet, cell phone or shoes.

Assuming the statistics presented are accurate, there is too limited of a sample size to extract information. For starters, assuming a class size of 300, there are 3 people at your school in the top 1%.
