If you scroll far enough back, there was a point in time where several TLSer's outed themselves in the fb group. I called and got it resolved. Some people (or, just me) got different forms. Instead of three forms, there were only two. Both scantron (one self history and one MMR with meningitis combined). -medical history form (the scantron/bubbly thing you can fill out yourself)
-immunization history form (must be filled out by your medical provider, its the one that talks about measles,mumps, rubelia)
-meningites form (you tell them whether or not you got vaccinated. if not, you just acknowledge that you know the risks
