1404 Transfer Motion Help

Does anyone have a link for a sample motion to transfer under 1404? The actual motions for transfer (as opposed to a memorandum in support) aren't available for viewing on WestLaw (at least at my school)...

This may not help, but I believe it's just a standard motion under rule 10 form.

It is helpful. But, yeah I assumed that.

I wanted to see it for the sake of content. It's unclear to me how much goes into the actual motion because its so often paired with a memorandum in support. In this case, its uncontested by all parties, so the motion itself is probably enough--I just want to make sure I have everything I need...
