NYU's "Dean Certification" and "College Questionnaire" Forms

Hello all,

I am in the midst of getting all my materials ready to apply to transfer to NYU, however, I see, as a requirement, I must send in a "Dean Certification" and a "College Questionnaire" form.

Where do I get/download these forms? What are they? Do I have to request them from NYU or my law school and undergrad?

Anyone that has transferred to NYU before, or anyone that has had a similar experience, please send some helpful advice... feeling kinda lost :shock:

I believe those forms are available from a tab on your active LSAC application to the school.
