1) I screwed around the first two years of UG before growing up and getting serious. Had a 2.9/3.0/3.8/3.8, graduating with a 3.3.
2) Applied to LS as a splitter with a 170+ LSAT, wound up at a t-30 school.
3) Killed it this year and am top 1-2%.
Should I write a quick GPA addendum for applications to HYS + Chi? Or does the upward trend speak for itself?

Your top 1% at a T-30 speaks for itself.
