i did. i tried calling, but was on hold for 10 minutes, so gave up. sent an email telling them i sent it in. to be fair, i sent it in late last week, so it may not have made it by august 15th, but should have reached nyu by now...

I got through after 15 minutes on hold to be told that they got my forms but that it hadn't been signed by a doctor. Which is not true, since I WATCHED the doctor sign it. When I told the woman this, she said "well your file is still being reviewed, so who knows." Umm, yeah.

I had the same issue, and it turned out that the doctor hadn't stamped the form with their address stamp, which is what caused the problem. Because that really calls into question its authenticity....

Also, has anyone else read the case for orientation? (Sorry if this has been discussed already). Its a bit weird that the first case we are reading is about some friends shooting their other friend in the face....
