I think if you take the form to the doctor and just tell him you had the pox you'll be ok. When we were young it was tons more common for kids to get it than not and get the vaccine eventually, so I'm sure he'll be fine with taking your account as fact. Still haven't got all my immunization stuff sorted out. How am I supposed to prove I had chicken pox? We were hardcore when I was little so there was no doctor involved, just lots of calamine lotion.
I also don't think I've had an "MMR2" shot...sigh - more to do before I arrive.
As for MMR2, you probably had it. It was required to start 7th grade in my district so that's probably likely everywhere else too.
But you should get that form in ASAP. Don't want them keeping you from starting classes.
