3L Conflicts Check

The firm I'm starting at in the fall just followed-up on the conflicts form I submitted more than six months ago asking for some more information about cases I worked on as a paralegal at a different firm more than four years ago. What's the best way to try to track this information down? I filled-out the form from memory as best I could, but of course I didn't have all of the information. Can I ask past firms for a list of matters I worked on? I'm not ever sure where to start here since it's been so long and the details the conflicts form asked for I just have no idea where or how to find.
