I'm a first-year who just started in biglaw and my group is crazy busy now.
But I have to take MPRE on a Friday evening. But it seems like Friday afternoon/evening is crazy busy for my group.
What is the best way to handle this? Should I tell them I won't be available for a few hours because of the exam? take a sick day? or reschedule to morning?

Tell people you’ll have to leave at 5 or whatever it is for the exam and then leave. I guess if people really needed you, you could log back in after the exam was over. Taking a sick day is both unnecessary and makes things worse because then your team is down a person all day. I suppose if you could take it on a weekend, though, that would be preferable, but more so you could avoid the stress….
