what's the standard in terms of medical leave for a surgery that requires potentially a month+ off from work?
is it "market" for people to paid that whole month and then maybe their bonus is prorated or something?

Bonus depends on firm. If you are at a reputable firm, I'm sure you have short term disability insurance which covers your salary.
