Legal-related Affidavits for NY Bar Application

I was a paralegal at two biglaw firms prior to law school and as I understand, I need to request a "legal-related" affidavit from each firm that I worked at to submit with my application for the New York State bar.

Has anyone had experience reaching out to old firms? If so, could you share the best method for doing so? I'm not sure if I should reach out to the recruiting coordinators or HR?
