Had a tough decision between 2 firms in 2 markets. Both great and very lucky to have received both of them. Accepted one yesterday and the other expires Monday. Having second thoughts, regrets, etc for not taking the other offer, but really don't think I can renege on my acceptance and I'm too afraid of the no-offer to try to split. My firm is a 10 week program and the other is a 12 week in the midwest.
Let's say the firm/city isn't all that everyone pitched to me. How should I go through 2L year and summer if I want to try to do 3L OCI? I know it's a shot in the dark and I'm still gonna try to focus on an offer at the firm i'm with, but i just want to keep this in the back of my head just in case.

Not sure what you're fishing for: isn't the answer 1) study, 2) maintain relationship with the other firm, 3) take SA seriously.

i guess i'm wondering when I tell the 2nd firm i'm going elsewhere, should i mention i'd like to talk to them over the summer about any 3L hiring they'll be doing? I know I've read that firms will sometimes say, "If you're interested, keep in touch with us during 3L" (or words to that effect), but I don't know how you'd raise that without them wondering why you didn't just take the current offer?

Rising 3L here who just accepted an offer with a different firm from my 2L summer.
If I were you, I would not contact anyone in the firm you're interested in until you have your offer from your first firm. I don't see any point.
Once you have an offer, or your 2L summer is up (which ever comes first), I would reach out to recruiting. I'd (1) Ask if they do 3L oci; (2) Mention I have an offer from my 2L summer firm; (3) I loved my 2L summer experience, but their firm still stands out to me for xyz; (4) You are considering other firms because practice & geographical interests have changed. You will be doing 3L oci, and would love to interview with their firm given the above reasons.

Did a partner make the offer? If so, call them to let them know that you accepted a different offer. Without being melodramatic, communicate that it was a tough decision, that they were your top choice in that region, but for personal reasons, you decided to go with the other region. This opens the door for the "let us know if you ever change your mind" speech. Enthusiastically, respond to that opening.
If you don't get it, let it go. If you decide you don't like where you've accepted, reach out in mid July to reconnect and ask about 3L hiring.

I think you made the right decision. You probably have a history of making solid decisions and it sounds like you thought about this one hard. It is common/healthy to question your decision, but be confident in your choice.
Relax. Don't RENEG. And move on with your life.
But of course to better gauge your situation I would need more facts (which city and firm you are going to be at (DC/NY/CHI/LA), which mid-western city and firm was the other firm, your family situation, long-term career aspirations... so many factors). Remember you know yourself best and are able to gauge your situation the best.
