Question for SAs: How much employment history did your law firm employment application form ask for? Did it ask if you had ever been fired or the reason for leaving previous jobs?

Similar question. A firm sent me these forms to fill out before my CB tomorrow. Not sure how many jobs I have to list or if the list of references should only be employers on this form.
