I think there is likely a fair mix of great and not great people at every firm.

So which firms routinely no-offered summers? or at least large percentage gets no offers?

All this data is available on NALP. Taking your list of firms over there will be far more efficient than people randomly listing firms that don't give 100% offers (of which there are a lot).
