Anyone got a sample email in which they let another firm that you CB'd with know that you have an offer on the table but you would like to hear from them?

How about
1. Sentence saying "hey, you told me to check in with you if anything changed" or something like that
2. Say you've got an offer, but haven't taken it because...
3. Reiterate the interest in their firm, saying you would like to consider any potential offer from them
4. Ask about a timeline
5. Say thanks and goodbye

Is it better to call a recruiter and let them know, or send an email?

Better to call the recruiter. But don't bother them unless the deadline on your other offer is a week or less away.
