I got a Milbank (J.H.) rejection via e-mail as well.

Has anyone gotten dings from Latham (J.H.), Paul Hastings (E.L.), Dewey (M.B.), or DPW (J.M.)? Should I assume I'm out of the running at those places?

i wouldn't assume you're out of the running until you actually have the rejection in hand, BUT i have heard of a few Latham rejections going out via snail mail. It depends on what office you applied for, as it seems different offices have been sending them out at different times (for example, i now that LA and DC Latham rejections have been sent out via snail mail; not sure about NY or Chicago though).
