Got an email from a recruiter, asking to email him back regarding scheduling a callback. I emailed him last weekend, and with my preferred dates, and didn't hear anything, so I emailed again today and still haven't heard anything. I haven't been scheduling for those two days in the event that I get scheduled with this firm then, but he still hasn't gotten back to me. Should I try again, or just go ahead and schedule interviews then, or still leave them open? I would really like to interview somewhere on those days.

Just call dude. You'll be able to figure it out in 5 minutes rather than stretching it out over email.

What is the policy for canceling scheduled CBs once booked?

Let them know ASAP, so that they can give the spot to someone else. 48 hours notice is preferable, but 24 is required. Within 24, go on the callback.

Please, please, please do not follow the advice in the last sentence. If you conclude you would not under any circumstances accept an offer at a firm where you have a callback, cancel the callback even if it's two minutes before it's scheduled to start!

I assure you that no lawyer wants to interview you if there's no chance you'll accept an offer at their firm. They will be very grateful to you for cancelling. And no firm wants to give you an offer that you are certain to reject. And if you let the firm make you an offer that you are certain to reject, you may be taking an offer away from one of your classmates who would be happy to accept it.

Obviously it would be better to cancel with more notice. But no matter when it is, if you decide there's no chance that you'd accept an offer from the firm, cancel the callback. Please.

If one is already in a secondary market where a firm who has neither accepted nor rejected you at this point is, is it ok to email recruiting saying can I come in and meet with some attorneys (i.e., can I have a CB) or just take their silence to you (and CBs of other people) as an implicit rejection?

In the real world you're right. But in OCS and law firm world, you are wrong. Better for your reputation to go, rather than a last minute cancel.

(I don't know nearly enough to answer your question. Probably telling them won't be a negative if you phrase it as "I'll already be there and wanted to tell you so you can save money if you were planning to fly me out." Though FWIW, the PW London office is doing their callbacks through NYC.)
