it seems like there may be a mythical hold pile but the vast majority of the holds turn into dings

I think a lot of the holds are due to biglaw partners being too busy to signature a rejection letter.

Do most firms send email confirmations of scheduled callbacks or are we supposed to just show up? When do people get their interviews' names and such?

Firms will send out confirmations roughly right after you schedule. You'll get the itinerary the day before. Some firms don't give you the names of your interviewers until you arrive at the firm, and some don't give them to you at all. (I remember at one firm they gave me a list of the interviewers' contact information at the very end, after the interviews were over.
In a case like that, it doesn't hurt to tell the interviewer that you didn't know s/he would be interviewing you until just a few minutes ago, before you launch into your "what practice group are you in?"-type questions.
