Just be honest.. Also, the explanation you gave for the "hazing" violation does not make any sense, so you may want to explain that more thoroughly when you're disclosing it to the schools.

Disclose, repent, move on. Your addendum should be about 1/10th the length of your post.

I appreciate the response! Part of the hazing code of student conduct at my school reads: "activities that...compel a student to: perform acts of personal servitude..." While there may be a debate about whether or not this applies to my actions, it is what it is. Thanks again!

I would include the language from the honor code in the addendum for the hazing violation.
