Short answer - Yes, GPA is taken at face value.
Longer answer - The things you listed will matter at the margins. In two otherwise identical applications, an applicant with a top Ivy UG / STEM degree / upward grade trend will likely be looked upon more favorably than one without. But in no way will a 3.5 from a top Ivy in Engineering be looked upon as equal to or better than a 4.0 from a state school in an easy major.
If you had a 1.7 freshman year, a GPA addendum may be in order.
