1. What's a good general format for a scholarship negotiation proposal? I was thinking something along these lines:
Hello,
I was offered this by your school. Recently I was offered this by other schools. I am a big fan of your school, and while I appreciate your already generous offer, if you could increase your offer it would make my decision easier. etc.
Edit - in my first draft, I briefly explained why I was considering all of the mentioned schools. Is that unnecessary?
2. Is sending a general e-mail acceptable, or should I attach the letter as a PDF?

Just don't overthink it. As long are you're polite and relay all the relevant facts (i.e., other school's offer, your strong interest in their school, request for scholarship increase, etc.), you're fine.
