What Schools Require a "Dean's Letter"

Hello everyone, I am trying to compile a list of schools that require dean's letters for disciplinary history at the school. I have a few dings on my record, and want to have them sent all together by my school instead of requesting this over and over again. I know that Duke and Columbia so far have requested it in the applications. I read online that Cornell requires it, but I did not see that in the application. Any others?

Duke NYU Columbia (I only checked from Chicago down to Georgetown)

Thanks! I just applied to all three and those were the only ones I saw. I just wanted to be sure I wasn't missing anything.
