Long story short, took the December 2015 LSAT and scored just under 150 due to my dog's death leading up to the exam. Decided to take the LSAT again in September 2016, improved by ~10 points.
Should I write an addendum addressing the lower score and how the death hindered my abilities?

LSAT addenda are never necessary. Your highest score will be the one considered.
Also, with all due respect, it's not a very strong excuse.
