My service organization was investigated for hazing and put on probation. Should I disclose this in an addendum? I'm unsure since this pertained to an organization I was part of and not me as a student.

Just call it a fraternity, imo. "Service Org" just sounds weird and is definitely just spin. Disclose it, I highly doubt that it'll make any impact in your cycle.

Wasn't a Greek organization.

On the odd chance anyone else has this issue-

I called my school and verified that disciplinary records for student organizations are separate from student disciplinary records. I went a step further and confirmed with the bar that they would only check student disciplinary records, if at all.

I can rest easy now.
