I wouldn't, unless the school is asking for one for an 8-pt jump. Saying you rushed and didn't study as much as you should have...twice...doesn't tell the adcoms anything flattering about you.

And if a school IS asking for one for an 8-pt jump, don't admit you were dumb about studying the first two times; just say you reevaluated and revised your study plan for the 3rd take.
