Addendum question

Hi everyone. I dealt with a serious medical issue throughout my junior year which hurt my GPA and made me have to withdraw for one semester. I have written a brief addendum to explain the situation that I am attaching to all my applications. However, some applications include a short space to explain GPA drops/withdrawal. For those applications, should I just use that space and not attach the addendum, use the space and attach the addendum, or just note in the space that I have attached an addendum to explain? I don't want to be redundant but I want to be clear about what happened.

Assuming the space doesn't have a character limit, I'd just copy/paste your addendum into the provided space. If it does have a character limit, then direct them to your addendum.
