Transcript sent without Request Form - Big deal?

Hi,

I requested an overseas university to send the transcript to LSAC. (Of course, sealed and stamped)
But I think they will send the transcript alone, without the Transcript Request Form.
Would this be a problem?

When I've requested mine sent in the past my university has had an "Address Line 1" and "Address Line 2" section.

For address line 1 I've put the address, and for 2 I put my LSAC #. This has worked out fine for me twice. Perhaps do something similar if you can?
